Foster Application
Fostering provides many homeless dogs with the opportunity for a second chance at life.  It also provides the ability to
rescue animals and place them in temporary homes until such time that a permanent adoptive family is found. You, as
foster parents, provide security, love, a place for these dogs to call home, and a place where the dogs have the feeling of
"belonging."

Our rescue experience has taught us that setting some basic rules and sharing tips as to what has worked for us will
help make the Foster Home experience safe and successful for the dogs we rescue, and for the humans and animals
who share their time and homes.  Before you sign on as a Foster Home volunteer, we ask that you review and agree to
the following rules:

I agree to the following:

  • that the dog(s) will not be placed in a dangerous situation (e.g. riding in an open pick up truck or left unattended
    in a car)
  • that the foster dog(s) will be kept in the house as a loved family member, and when not directly supervised by the
    foster family I will keep the dog(s) in a secure area, that will ensure its safety and the safety of my family, my
    animals, and my property
  • that I will feed, water, groom, exercise, and socialize the dog(s) as appropriate
  • that I will not allow the dog(s) off leash except in a secure fenced area of my property, and that I will never use
    chains or tie outs under any circumstance
  • that I am a foster home and specific veterinary care is approved by CSTMR. My volunteer responsibility may
    include transporting the dog(s) to veterinary appointments and administering any medicines or treatments
    ordered by the vet
  • that I will not be reimbursed for daily board and food for the foster dog(s)
  • that I will never administer physical punishment or physical discipline to the dog beyond a light collar correction,
    and that if there is a problem beyond my capability, I will immediately contact the Director of CSTMR so that
    arrangements can be made to remove the dog
  • that I will call the Director with any questions or problems
  • that CSTMR will not be responsible for illness or injuries to people or pets or for loss or damage to property
    resulting from the failure to follow these rules
  • that the animals remain the property of CSTMR and that I will surrender the dog(s) to a CSTMR representative
    immediately upon request
  • that a “choke”  or “prong” collar will not be used
  • that veterinary care will not be reimbursed unless preapproved by the director of CSTMR

I agree with all of the above principals set forth by Colorado Shih Tzu Maltese and Lhasa Apso Rescue (“CSTMR”).  
Furthermore, I will adopt and implement the above principals while caring for animals entrusted to me by CSTMR.

Because your new little friend is unfamiliar with your home and your habits, he/she must be kept inside to prevent
him/her from wandering off.  Additionally, sometimes dogs require to be either crated or confined to a certain area during
the hours you are not home.  This should be discussed with the rescue director when special circumstances are
required.  If you have other pets, they should be current on vaccinations.  

The CSTMR is always grateful for the help you provide when you foster one of its homeless dogs.  As you foster an
animal, we will always be available to you for assistance.  We can, and will provide the knowledge and expertise to help
you with the care, welfare, and training of your foster pet.  Several phone numbers are made available to you so that you
never need to worry about being alone without answers to your questions.

We provide the cost of veterinary care through our veterinarians and clinics for your foster pet.  This also includes
vaccinations and testing for diseases, as well as spay/neuter.  At times, we are able to offer supplies such as a carrier,
crate, temporary ID tag, collar, leash, or food and water bowls. This will depend largely on the donations that we receive.  
To provide the best foster match for you and your home, we do require an application and a home visit.  In doing so, we
make sure it’s a winning situation for both you and the dog you want to foster.  If you take in a foster pet and it is not
working for either you or the pet, we will take it back into our care and find a more suitable foster home. We want the best
situation for everyone (furry and otherwise) involved.
